Covering the Illawarra and South West Sydney regions, our Support Coordination service is delivered by highly experienced and skilled Coordination of Supports professionals with over 20 years of combined complex case management, individual advocacy

and disability services experience.

 

Our Support Coordination service is focused on assisting NDIS participants to work towards reaching their goals and getting

the most from their NDIS plan.

 

Every aspect of support coordination is covered with participants, including:

 

  • Setting up My Place online NDIS Participant Portal

  • Training on the most effective use of your My Place account

  • Unpacking the NDIS plan and developing a clear action plan for Support Coordination which forms part of the Service Agreement

  • Dealing with all providers and ensuring that quality and safeguards are delivered

  • Addressing all points of crisis

  • Building the capacity of participants and their representatives to best understand the NDIS and coordinate their supports in future

  • Keeping you informed every step of the way, maintaining regular contact and communication

  • Prepare for the plan review

  • Assist in gathering evidence for potential unscheduled review
